Understanding Your Itemized Bill

An itemized hospital bill is essential for reviewing charges in detail, enabling you to spot potential errors or unnecessary expenses. Unlike summary bills, a true itemized bill includes CPT and HCPCS codes—standardized medical billing codes that reveal each procedure and service. These codes let you cross-reference each charge, detect duplicates, and identify any services you may not have received.

How to Obtain Your Itemized Bill:

 

​Hospitals typically don't provide itemized bills unless requested, but they are legally required to send one within 30 days of your request. Patient Overwatch can request this on your behalf, streamlining the process so you can focus on understanding and potentially reducing your bill. You can also provide access to us via MyChart, Cerner EHR app or other EHR accesses.

What is a UB-04 and Other Types Bills?

What is a UB-04?

The UB-04 is the standard claim form hospitals use to bill insurance providers, including specific details about services provided. This form is invaluable for Patient Overwatch, as it provides extra details, such as CPT modifiers, that enhance our review accuracy. For patients admitted to the hospital, it also includes diagnosis-related groups (DRGs), which insurance companies use to determine payment rates.

Consolidated Summary Bills

A consolidated summary is the more generalized bill hospitals often mail out, summarizing procedures and charges without the detailed breakdown found in itemized bills. These summaries may obscure billing inaccuracies or inflated charges.

Professional Bills

If you were treated by specific doctors or specialists during your hospital stay, you'll likely receive separate professional bills for their services. Currently, Patient Overwatch focuses on hospital bills but plans to extend support to professional bills in the future.

  1. Conversation: we have a personal conversation with you about your situation and go over how we may be able to help then we send you secure agreements to review and sign to get started.

  2. Request Records: We obtain your itemized bill, your health record history, explanation of benefits (health insurance) and UB-04 form.

  3. Thorough Review: Our expert coders and AI-driven software analyze every line item, checking against medical records and industry standards to flag errors, overcharges and quality issues.

  4. Negotiation: After identifying discrepancies, we create a detailed negotiation letter by one of our attorneys and advocate with the hospital on your behalf.

  5. Results and Settlement: We provide you with the negotiated balance. If you agree, Patient Overwatch can even handle the payment arrangements.
